Plot: Movie begins with hero being called in some Police HQ, either he is spy or some police agent. There, police chief tells him that he is required to visit some other country on some work and that he will take the help of a woman colleague. Chief hands him eurorail (If I am right) tickets. At this point, Hero asks that why can't they travel by air, chief responds by saying that It's too risky and that you'll be identified and send to jail in the country where you land. Chief further tells the hero about location of woman collegue. Hero goes to the location and meets her. Movie continues and it is shown that after that hero goes to meet his wife (A Paramedic/Nurse by profession) and tells her that he needs to go on a project. Wife rebukes by saying that he was supposed to spend time with her and it would not be right for him to leave her. Few seconds later hero manages to convince her and she punches him casually on his left shoulder.
Hero bids his wife bye and continues with the woman colleague on the train. Hero proceeds to platform with colleague where the security checks passports of both him and her. The security guy grows suspicious. He has in his hands, photos of suspects but woman colleague is in disguise. Security guy casts repeated glances on woman colleague. At this moment, hero is almost ready to fight because he assumes that security guy has figured out who she is, but everything goes without a problem and they board the train.
Meanwhile hero's wife tries to surprise his husband by joining him in the journey but not telling him that she would be on the train. She plans that she would visit her husband during the journey and meeting him at one exact moment (Probably hero's birthday was falling on the date when he would travel to complete his assignment, and wife was forcing him to not go away because she wanted to spend time with him on his birthday.) She brings along with her two of her kids (A girl and a boy).
In the train compartment where hero and colleague are resting it is shown that colleague is overreacting flirt. She starts coaxing the hero telling him that she is preparing a drink for him. Hero tells her to just stay away and that he can take care of himself. She goes too close to him. At this moment Hero's wife enters the compartment with kids to surprise her husband. Her son holding a camera in his hands enters first screaming "Daddy, Daddy" only to find his daddy perched in arms of another woman. Wife sees all this too. Hero introduces colleague to wife. Wife passes sarcastic comment that colleague name doesn't match with her character.
While all this is happening it is shown that a gang of criminals have got hold on bio-engineered deadly virus that is destined to kill many. But a fight ensues and one of the woman member of gang ends up dropping the steel case in which virus serum is confined, in effect opening the case. It is shown that by opening the case virus is released and it travels to other compartments travelling through ventillation system. Everyone in train starts getting sick. Criminals manage to take hold of all passengers in captiviy.
Movie continues and it's shown that wife and hero are separated into different compartments on train. Wife ends up in compartment full of sick passengers and criminal gang. Wife tells sick guys that your symptoms match with that of small-pox. A young guy talks to her and says that "I think this was eradicated long time back". Wife helps everyone in compartment by taking to them drinks to avoid fluid loss.

Ask Ques.SciFi Watchers - Name of movie on Biological Warfare.?
Typed in "smallpox" and "train" on imdb.com plot summary search and came up with Derailed. It also mentions a guy having to take action on his birthday, so...
